WHAT IS PERSONAL INFORMATION AND WHY DO WE COLLECT IT?
Personal information is information that identifies you as an individual. Personal Information collected includes: your name, date of birth, address, contact details, and medical information.
This personal information may be obtained via the Mynd Naturopathy website, email/phone/social media correspondence, registration forms, and during consultation/s. In some circumstances, personal information may also be collected from other sources including information from your guardian, and other healthcare providers such as allied health professionals, specialists, community health services and pathology and diagnostic imaging services.
Our primary purpose for collecting your personal information is to provide healthcare services to you. We may also use your personal Information for secondary purposes closely related to the primary purpose, in circumstances where you would reasonably expect such use or disclosure. If this is the case, consent will be sought before proceeding.
When collecting your Personal Information, we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.
Please note; you have the right to deal with us anonymously or under a pseudonym unless we are required/authorised by law to deal with identified individuals.
SENSITIVE INFORMATION
We understand that much of the information you will disclose during your consultation may be of a highly personal and sensitive nature. As such, this information will be used by us only for the primary purpose for which it was obtained. On occasion, this information may be used for a secondary purpose that is directly related to the primary purpose or disclosed to third parties with your consent, or where required or authorised by law.

THIRD PARTIES
Where reasonable and practicable to do so, we will collect your Personal Information from you only. However, in some circumstances, information may be provided by third parties. In such a case, we will take all reasonable steps to ensure that you are made aware of the information provided, and by whom provided it.
SECURITY & DISCLOSURE OF PERSONAL INFORMATION
Your Personal Information is stored electronically in a manner that reasonably protects it from unauthorised access, modification or disclosure. When your Personal Information is no longer required for the purpose for which it was obtained, we will take all reasonable steps to destroy/permanently de-identify your Personal Information. Please note; Personal Information will be stored in client files and kept for a minimum 7 years as required by law.
Disclosure of your Personal Information will only be done in the following circumstances; to third parties where you consent to the use or disclosure, or when it is required or authorised by law. This includes, but is not limited to court subpoenas, when necessary to lessen/prevent a serious threat to client’s life, health or safety, or that of public health and safety, when there is a statutory requirement to share certain personal information.
ACCESS TO PERSONAL INFORMATION
You have the right to request access to, and correction of, your personal information. If you wish to access your Personal Information, please contact us in writing. Mynd Naturopathy will not charge any fee for your access request but may charge an administrative fee for providing a copy of your Personal Information. Please allow up to 14 days for release of information.
MAINTAINING THE QUALITY OF YOUR PERSONAL INFORMATION
It is important to us that your Personal Information remains up to date. We endeavour to take all reasonable steps to ensure your information is accurate and complete. From time to time, we may ask you to verify your Personal Information. If you find that the information we have is not up to date or is inaccurate, please advise us as soon as practicable so we may update our records and ensure to continue to provide quality services to you.
This Privacy Policy may be updated to ensure compliance with Aotearoa Privacy Act 2020/Privacy Principles and Australian Privacy Principles (APPs).
